Enrollment is simple and can be completed in approximately 30 minutes per child. Please make sure to watch our introductory video, found on our homepage, before proceeding.
By clicking the "Start Enrollment" button below, you will begin the enrollment form, which is immediately followed by our payment page. You must complete a separate enrollment form for each student you wish to enroll. Payment is required at the time of form submission; you will be responsible for the cost of enrollment and any additional services that you have selected. Service descriptions and corresponding prices can be found on our Services page and our current enrollment rates are:
1st Time Enrollment: $105 (K-8); $120 (9-12) Re-enrollment: $85 (K-8); $95 (9-12)
We offer year-round enrollment! You are welcome to enroll with Applewood any day of the year. We operate with a 365-day school calendar, meaning that your child's school year will extend one full calendar year from when their enrollment is finalized. You will be due for re-enrollment on the same day each year and have the same "official start date" each year.
Our online enrollment form is easy, secure, and HIPAA-Compliant. Please make sure you have the following information available before proceeding to the enrollment form. These will all be required during the enrollment process!
Basic demographic information for you and your child
Valid permanent mailing address
PDF copy of your child's current Florida School Entry Health Exam, Form DH 3040 (obtained from their healthcare professional)
PDF copy of your child's current Florida Certificate of Immunization, Form DH 680 (obtained from their healthcare professional)
PDF copy of your child's birth certificate
Valid payment method (PayPal, Visa, Mastercard, Discover, or American Express)
